MOUD Toolkit Evaluation Consent

 

The Southwest Interdisciplinary Research Center (SIRC) at Arizona State is evaluating the medications for opioid use disorder (MOUD) Toolkit & Course. We are seeking input from individuals age 18 and over who have completed the course. Participants will have the opportunity to download a certificate of completion for the course at the end of the survey.

The purpose of this survey is to understand participant knowledge, satisfaction, actions and/or behaviors since completing the course. Your responses are critical to improve this course and inform future courses on MOUD.
   
Your survey responses are anonymous and the information you provide in the survey will not be linked to you. Your participation is completely voluntary and you can choose not to answer questions that make you feel uncomfortable. If you agree to participate, click on the next button below and the five-minute online survey will begin. 
   
Your responses will be anonymous. The aggregate results of this study may be used in reports, presentations, or publications but no identifying information will be used.
